Please give name(s) and contact details of those who will be present throughout the event and will be responsible for the organisation of the event and health and safety at the event. These could be (1) Organiser); (2) Duty Officer; (3) Referee.

Set-up - give names of those (suggested 3) responsible for setting-up the hall and when this will be performed, bearing in mind that it must by after 4pm if on school days and that key access is required.
